Herniated Disc Surgery on Olympic Gymnast
Scott is one of the top gymnasts in the world. He suffered a herniated disc in a training accident. He went to Dr. Delamarter who rebuilt his back with the pedicle screw, which is sometimes used as an adjunct to spinal fusion surgery, it provides a means of gripping a spinal segment.
The screws themselves do not fixate the spinal segment, but act as firm anchor points that can then be connected with a rod.
The screws are placed at two or three consecutive spine segments (e.g. lumbar segment 4 and 5) and then a short rod is used to connect the screws. This construct prevents motion at the segments that are being fused. The treatment allowed Scott to compete for the next Olympics within one year.

Dr. Bruce Hensel Reports on the Artificial Disc Replacement (ADR) – The Spine Institute Santa Monica
The Spine Institute in California has done more PRODISC® replacement surgeries than any other institution in the US.
What is it? The FDA Approved PRODISC®-L Total Disc Replacement is an artificial intervertebral disc made from metal and plastic that is used to treat pain associated with degenerative disc disease (DDD). DDD is defined as discogenic back pain (pain resulting from a degenerated intervertebral disc) with degeneration of the disc confirmed by patient history and radiographic studies. The PRODISC®-L Total Disc Replacement is implanted to replace a diseased or damaged intervertebral disc during a surgical procedure called spinal arthroplasty.
How does it work? The PRODISC®-L Total Disc Replacement consists of three parts:
1. Two metal (cobalt-chrome alloy) endplates that are anchored to the top and bottom surfaces of the spinal bones (vertebrae)
2. A plastic (ultra-high molecular weight polyethylene, or UHMWPE) inlay that fits between the two endplates
The plastic inlay and endplates help restore the natural distance between the two vertebrae (disc height). The top (superior) endplate can slide over the domed part of the inlay, which can allow movement at the level where it is implanted.
When is it used? The PRODISC®-L Total Disc Replacement is indicated for spinal arthroplasty in patients who:
1. are skeletally mature
2. have degenerative disc disease (DDD) at one level in the lumbar spine (from L3-S1)
3. have no more than Grade 1 spondylolisthesis at the involved level
4. have had no relief from pain after at least six months of non-surgical treatment
What will it accomplish? The PRODISC®-L Total Disc Replacement is used to replace a damaged intervertebral disc. The device may restore disc height, may reduce pain, and may allow movement at the level where it is implanted.

Cervical Disc Cage Fusion (pre-2000 News Clip)
Cages have been used since 1992 to help fuse lumbar vertebra. Interbody cage fusion uses a hollow threaded titanium or carbon fiber cylinder to fuse two vertebrae together. The diseased disk is removed and two interbody cages are placed in the opening where the diseased disk has been removed. The cages are filled with bone graft. The bone grows through the holes in the cages fusing the vertebrae.
How does it work? The fusion cage component maintains the spacing and temporarily stabilizes the diseased region of the spine, while the InFUSE™ Bone Graft component is used to form bone which would permanently stabilize (fuse) this portion of the spine.
When is it used? The device is used in the lower region of the spine (L4-S1) to treat degenerative disc disease.
What will it accomplish? A clinical study showed that the use of this device was as safe and effective in promoting spinal fusion as the same fusion cage component filled with autograft bone.
